What is Thermage?

A new non laser procedure for tightening the skin uses radiofrequency to heat the dermis and potentially the subdermal tissues. Thermage is FDA cleared for the non invasive tightening of rhytides using this proven mechanism of tissue tightening.

Radiofrequency tissue tightening is a recently introduced complement to and in some applications, alternative to non ablative laser technologies.

This device uses a novel form of radiofrequency energy to create a uniform field of dermal and even subdermal heating,while contact cooling protects the epidermis. This device can safely deliver higher energy fluences to a greater tissue volume than non ablative lasers.

Patient Selection

Guidelines should include treating a broad surface area at appropriate fluences and carefully selecting patients in their thirties to seventies,who have medium quality skin thickness and mild to moderate jawline and neck laxity. All skin types can can be treated effectively as dermal heating while simultaneously cooling the epidermis with no hyperpigmentation or hypopigmentation. Patients with advanced photoaging or more severe skin sagging benefits from this treatment.

Treatment Approach

It is capable of tightening the skin and improving the contours. The physician must analyze the patient’s three dimensional facial structure to assess those areas that would benefit most from tightening. Typically this would include the forehead /brow area as well as the lower cheek,jawline and submental region.

Once this analysis has been done and the treatment plan created, therapy can be initiated. By varying the fluences used and the number of passes in each area of the face, the physician can preferentially tightten some areas,reduce the prominence of others(i. e. jowls) and overall improve the shape and appearance of the face.

Patient Interviews

Patients scheduled for treatment must be in general good health, and must not have a pacemaker. It is recommended that pregnant women should not be treated. It is also important to assesss the following pretreatment:

Do you have any important social events coming up?
